| Official transcripts form previous schools if high school student Transcripts should be mailed directly from the school |
| Official copy of courses in process if the student is transferring during the semester |
| Completed medical record form |
| Before the student can be formally enrolled, an interview and /or parent conference is required. |
| A student who has attended non-English speaking school and is not a native English speaker is required to take the Student |
